UTAH MEDICAREMADE EASY
D-SNP plans are a type of Medicare Advantage designed for people who qualify for both Medicare and Medicaid (dual eligibles). Eligibility and plan availability vary. We do not guarantee eligibility.
General information only — not a recommendation of any plan. Plan availability, costs, and benefits vary by county and eligibility.
A Dual Eligible Special Needs Plan (D-SNP) is a Medicare Advantage plan for people who have Medicare and also qualify for Medicaid (full or partial). These plans coordinate with state Medicaid programs more closely than standard Advantage plans.
Only a review of your situation can determine whether you qualify — this page cannot confirm eligibility.
Medicare is generally the primary payer for Medicare-covered services; Medicaid may pay second for certain costs and services. Medicare Savings Programs (MSPs) may help pay Medicare premiums for people with limited income. Ask about MSPs if you have Medicare and limited income.
D-SNPs range from basic coordination to more fully integrated models (including FIDE-SNPs in some markets). Availability depends on state and county.
Many dual-eligible individuals qualify for a monthly Special Enrollment Period, which can allow joining or switching certain D-SNP plans more often than standard AEP rules — rules depend on your dual status and plan. You must continue to meet eligibility to stay enrolled. We can help you explore whether a D-SNP available through carriers we represent might fit; we do not offer every plan and cannot guarantee approval or savings.
